Referrals & Admissions

For almost 50 years, CRi has supported people with mental health needs & developmental disabilities where they live, work, & play. We would love the opportunity to learn more about your family.

Choice. Respect. independence.

CRi has an array of supportive housing options to fit all levels of need & utilizes a centralized Referrals & Admissions process for all programs & locations. To request service(s), download & complete the referral form.  

To have an Admissions team member contact you directly, please complete our online referral request information form.  If you have any questions, please contact Ricky Yates, Director of Admissions and Social Work at ryates@mycri.org.
