Permanent Supportive Housing 

The CRi’s Permanent Supportive Housing program helps people move into safe, stable housing while receiving support that meets them where they are. This program is built around CRi’s belief that everyone deserves choice, respect, and independence. It supports individuals who may be at risk of homelessness, transitioning from a state psychiatric hospital, moving out of long-term homelessness, or ready to take the next step toward living more independently.

Support That Helps People Stay Housed

Permanent Supportive Housing is not just about helping someone find a place to live. It is about helping them feel supported, connected, and able to maintain that housing over time. CRi staff work directly with each person to understand what they need, what challenges may be getting in the way, and what support would help them feel more stable. Every person’s situation is different, so support is shaped around their goals, needs, and level of independence.

Support may include:

  • Help during the move into housing

  • Coordination with landlords and housing providers

  • Connection to community services and resources

  • Ongoing support to help maintain housing stability

  • Help building independent living skills

  • Collaboration with care teams, families, and community partners when appropriate

Moving Toward Greater Independence

The goal of Permanent Supportive Housing is to help each person maintain safe and stable housing while continuing to build confidence, independence, and connection within the community. With the right support in place, individuals can take meaningful steps toward a more stable and fulfilling life.
